Service Nomination

CAHF's Distinguished Service Award

The Distinguished Service Award is designed to recognize individual CAHF members – both providers and associates – who have demonstrated exceptional commitment to the goals of CAHF and fostered the highest professional ideals.

All nominees will be judged on the following criteria:

Active involvement in long-term care (at least 10 years)
Active involvement in the community/civic programs
Active involvement in CAHF at the chapter level
CAHF member for at least 10 years

Winning entries will receive:

Recognition at CAHF’s Summer Conference
A framed Distinguished Service Award Certificate

All nominations must be submitted online by EOB on Monday, April 15, 2024.

Judging will be conducted by the CAHF Board of Directors. Award winners will be notified within 4-5 weeks.
