DescriptionThis presentation offers a basic overview of how POLST and Advance Directives are utilized within long-term care facilities, focusing most specifically on the POLST form. Discuss the content of the POLST, the implications behind each section, and the logic behind the POLST; which includes when the document is and is not appropriate for use with an individual resident. 

Verbalize what a POLST form is
Differences between a POLST and an Advance Directive.
Identify when an Advance Directive and/or POLST form is appropriate for a patient
Understand the different sections of POLST form
Understand the importance of Advance Care Planning conversations.

CE Hours:
Registrants must be individually logged in for the full training to receive CE hours (NHAP/P - 1 and BRN - 1)

BOARD REFUND POLICY: In order to receive a refund, cancellations for QCHF/CAHF classes must be made five (5) business days prior to the beginning of the course. Cancellations must be in writing to cmerced@cahf.org or via FAX at 916-446-4454. Transfer of registration is done ONLY if notified in writing prior to the start of the class.
