Description: This presentation offers a basic overview of how POLST and Advance Directives are utilized within long-term care facilities, focusing most specifically on the POLST form. Discuss the content of the POLST, the implications behind each section, and the logic behind the POLST; which includes when the document is and is not appropriate for use with an individual resident.

Understand the different sections of POLST form
Understand the importance of Advance Care Planning conversations.
CE Hours:
Registrants must be individually logged in for the full training to receive CE hours (NHAP/P - 1 and BRN - 1)
BOARD REFUND POLICY: In order to receive a refund, cancellations for QCHF/CAHF classes must be made five (5) business days prior to the beginning of the course. Cancellations must be in writing to cmerced@cahf.org or via FAX at 916-446-4454. Transfer of registration is done ONLY if notified in writing prior to the start of the class.
