The cost of a resident fall in a skilled nursing facility costs on average $9,100 - 13,500, according to the Centers for Disease Control and Prevention. In 2008 Empira, a Minnesota consortium of older adult service providers, applied for and received funding from the Minnesota Department of Human Services Performance Incentive Payment Program to prevent and reduce resident falls in fifteen of their skilled nursing facilities. The Empira fall prevention program is a combination of nationally recognized, evidence-based fall prevention practices and practical applications from the most recent research findings. Empira, however, challenged many of the standards of practice for reducing falls.
Their program outcomes included skilled nursing facilities that are now alarm-free and restraint-free, with corrected bed heights (no low beds), no floor mats, and the cautionary use of gripper socks. While this was a “nursing led” program, it was not a “nursing only” program. It required the skills, knowledge, participation, and commitment of all employees.
At the completion of their 3-year program, Empira's skilled nursing facilities collectively achieved an average of 30% reduction in the prevalence of resident falls. These findings are also coupled with a significant decrease in the number of recurrent falls and falls with injuries.
During her presentation, Sue Ann Guildermann will provide an overview of the implementation of a comprehensive fall prevention program. A key to the success of this fall prevention program was the ability for all staff to conduct a post-fall root cause analysis. They were able to analyze the internal, external, and system conditions and operations which may have contributed to residents' falls. Interventions that were used as a result of the post-fall root cause analyses will be discussed, as well as the outcomes that were learned from this program. The session will conclude highlighting the practices and actions taken by facilities who continue to sustain this fall prevention program and have moved into the implementation of their new program, improving the quality of sleep and wake.
This fall reduction program was the foundation and precursor for Empira's current three-year government funded project: to improve the quality of sleep hygiene of their residents. They are investigating and eliminating the causes of sleep and wake disturbances of residents in skilled nursing facilities.
As Empira progressed through their fall prevention program, they kept their state CMS surveyors informed of their findings and outcomes. Minnesota state surveyors continued to support the efforts and interventions of this program to such an extent that they invited Empira staff to present and educate the various CMS state and federal surveyors at conferences and seminars. Empira staff continue to travel nation-wide providing education to both providers and surveyors within the long-term care skilled nursing facility profession.
Objectives:
This session will provide an overview to the implementation of a comprehensive fall prevention and alarm elimination program. At the completion of this session, attendees should be able to:
- Explain root cause analysis
- Identify the three types of root causes for resident falls in skilled nursing facilities
- Describe the three categories of interventions from the National Center for Public Safety
- Select appropriate interventions to prevent and/or reduce falls in skilled nursing facilities
