CAHF AWARDS PROGRAM
One of CAHF's priorities is to ensure that long-term care providers are recognized for their excellence. Through our annual awards program we honor providers, dedicated staff and volunteers for their achievements and their dedication to long-term care.
Awards are presented at CAHF's Spring Legislative Conference, Summer Conference and Annual Convention.

Showcase Awards

The Showcase Awards are designed to recognize facility, company, chapter or regional efforts promoting the positive image of long-term care. CAHF's Showcase awards are presented at the Legislative Conference in Sacramento and will recognize community events in the following four categories:
- Community event
- Media event
- Grassroots effort
- Social media — new in 2011

SUMMER AWARDS – Presented at CAHF's Summer Conference
Distinguished Service Award

The Distinguished Service Award is designed to recognize individual CAHF members – both providers and associates – who have demonstrated exceptional commitment to the goals of CAHF and fostered the highest professional ideals.

WINTER AWARDS– Presented at CAHF's Annual Convention
Excellence in Programming

The Excellence in Programming Awards recognize individual facilities for superior achievement in key areas of long-term care, such as activity programs, community involvement, family involvement, quality assurance, resident councils, special services and staff development.

Facility Super Stars
|
|
The Facility Superstars Awards recognize employees of CAHF member facilities who have 10 or more years of service and have dedicated their lives to long-term care. The top 20 nominees are selected by CAHF's Board of Directors to receive the award and attend the awards ceremony.

Thomas E. Spindle Award
|
|
CAHF's most prestigious award, the Thomas E. Spindle Memorial Award recognizes an association member who has devoted considerable time and knowledge and provided exemplary service to CAHF at the facility, chapter and state-wide levels.

HOW TO SUBMIT NOMINATIONS:
General Nomination Requirements:
- Nominations must be submitted using the forms and specifications provided by CAHF. Additional information and/or materials will not be considered.
- Nominations can be submitted in electronic format (MS Word files) or in hard copy (paper) format. (See instructions below.)
To Submit Your Nomination Electronically
To complete and submit your nomination electronically using e-mail and Microsoft Word:
• Right click on the MS Word version of the nomination form above. Select "Save Link As" to save the form as a Word document to your computer. Save the form under the nominee's last name and first initial or program name. (Example: SmithJ.doc or SeniorGames.com)
• Open the document you have saved in Microsoft Word and complete the nomination form by filling in the form fields as indicated.
• When you are finished, save and close the document.
• Open your e-mail program. Use your e-mail program to send the document as an attachment to jrye@cahf.org or save it to a disk or CD and mail it to the address below.
To Submit Your Nomination in Hard Copy (Paper) Format
Hard copy (paper) nominations will also be accepted. Click on the appropriate nomination form below. Complete the nomination, then fax it to Kelley Queale at (916) 441-6441, or mail it to CAHF, 2201 K Street, Sacramento, CA 95816.
